*Valid Blood Test From The Last 6 Months Indicating HIV, Hepatitis B & C Status

*Please Note: In order for Louisiana Wrestling Alliance to consider you to be apart of future events a valid blood test from the past 6 months must be submitted along with your Professional Wrestling Contestant License from the Louisiana State Athletic Commission. Both must be submitted. No exceptions.
